If you complete an objective in TFE with a Mission Complete message and then exit to DOS your progress isn't saved.
Test:
in TFE, rescue Madine and immediately exit to DOS without pressing Next Mission. When you start TFE next time you are still on Detention Center
In DOS if you do the same as above your progress is saved and you can play on Ramsees Hed.
